PA Disclosure Statement or FBI Background Check

PA DISCLOSURE STATEMENT or
FBI BACKGROUND CHECK

ONE OF THESE IS rEQUIRED FOR ALL volunteers.  see below.
DISCLOSURE STATEMENT:

Only for unpaid volunteers who have lived in the state of Pennsylvania for the entirety of the previous ten-year period.

Please read, sign, date, and bring to the church office.  If you have lived in Pennsylvania for the past ten consecutive years, that’s it!  If not, see required FBI Background Check (fingerprinting) below.
FBI CRIMINAL HISTORY BACKGROUND CHECK:

Only for unpaid volunteers who have NOT lived in the state of Pennsylvania for the entirety of the previous ten-year period.

There is a cost of $23.25 for your fingerprinting and background check.  The church will reimburse your cost.  Please see Step 8 for reimbursement instructions.

Step 2. Right side of page: Find an IdentoGO Center: enter your zip code

Step 3. Choose location near you and click on it

Step 4. Click ‘Schedule Appointment’

Step 4. Bottom of page click on ‘Digital Fingerprinting’

Step 5. Enter Service Code: 1KG6ZJ

Step 6. Click on ‘Schedule or Manage Appointment’

Step 7. Complete information as required

Step 8. Pay the fee of $23.25. When you receive your confirmation you MUST either print that page out and bring to the church office or email the church at info@crosscreekcc.org and the church will reimburse your money back to you.

Step 9. Refer to chosen location to make an appointment.

Expect to receive your clearance certificate in the mail within 1-2 weeks.
